DATA PRIVACY STATEMENT (7.5.18, 13:30)
As part of the cooperation between the patient and the osteopath and physical therapist Michaela Liedler, MSc D.O., personal data will be collected. These data include the patient´s full name, address, telephone number and email address, and will be used for patient contact and for billing. To facilitate contact, the name and telephone number of each patient will be saved to my mobile phone contact list, although any patient who does not wish their data to be saved on my phone will have their data removed upon request. After treatment has ended, all patient-related data will be deleted from my phone.
This data will only be used for purposes of billing and will not be divulged to third parties.
Personal data collected in the course of treatment are used exclusively for their intended purpose and I will personally shred and dispose of them after the end of the treatment cycle.
These data will not be divulged to third parties. Should the situation arise where patient data could be useful for treatment in cooperation with other professionals, the patient will be informed and asked for their consent.
Further information to personal data and data privacy
Patient history and files are stored securely in a locked file cabinet in my home. (11.5.18; 17:38) The key for this file cabinet is in a drawer in the kitchen.
All files located in my office are either under my supervision or in a locked drawer in the treatment room (18.5.2018). The key for this drawer is carried on my person.
The data which are saved in the cash register are protected by a password. My mobile phone and computer are also protected by passwords.
The photos on my website are all used with the explicit consent of the patients pictured. The photos are stored on a password-protected computer and an external hard drive which is kept in a locked cabinet.
No data will ever be divulged to third parties. Should the situation arise where patient data could be useful for treatment in cooperation with other professionals, the patient will be informed and asked for their consent.
Patient data forms will be shredded and disposed of after the end of the treatment cycle, and all patient-related data will be deleted from my computer. Personal data collected in the cash register will be deleted after the mandatory storage period of seven years.

All data collected via the contact form will be used exclusively for patient contact regarding treatment or participation in seminars and for billing via HelloCash. These data will be stored for seven years, unless treatment did not take place or participation in the seminar was cancelled, in which, the data will be deleted sooner.
Subscription may be cancelled at any time.
By contacting Ms. Liedler, I hereby grant her my consent to use my telephone number and email address to contact me via email, phone, text message or WhatsApp in the future. In the event that I schedule an appointment, I hereby grant my consent that my contact information (name, email address, phone number, date of birth) be used for billing.
I grant my consent that my email address be used for MailChimp.

Terms and conditions
Scheduled appointments are to be upheld in a punctual manner.
Appointments which cannot be upheld must be cancelled at least 48 hours in advance (a text message or voicemail will suffice), or the planned treatment will be billed in full. This allows other patients on the waiting list a chance to receive an appointment.
After treatment, a number of reactions to therapy are possible. These can be physical reactions in the form of pain (such as bruising, aches, sore muscles) or vegetative reactions (such as sleepiness, nausea, dizziness, light-headedness/exhaustion). These reactions usually appear within the first four days after treatment and subside quickly.
Complications rarely occur, nonetheless, I would like to inform you of the theoretical possible complications of therapy, such as hematomas, injuries to muscles, tendons or ligaments, broken bones, dislocation, vascular or nerve damage.
All treatment techniques are oriented toward the personal boundaries of the patient and it is the responsibility of the patient to speak up should a technique become uncomfortable or too intense. Each technique can be modified, changed and/or interrupted at any time.
If at any time during treatment you become worried, or if possible reactions persist two weeks after treatment, please contact me via SMS and I will get back to you right away!
